The Local Map Pack: How to Get Your Practice Into the Top 3

By Lonnie Sanders III · June 20, 2026

The local map pack, sometimes called the three-pack or the local pack, is the block of three business results that appears at the top of Google search for local queries. For a query like "orthopedic surgeon near me" or "chiropractor in Emeryville," those three results capture the majority of clicks. Getting into it is not optional if patient growth is your goal.

What determines who gets in.

Google's local ranking algorithm evaluates three main factors:

Proximity. How close is the business to the searcher's physical location? You cannot change this. A patient searching two miles from you will see closer competitors ranked higher, everything else equal.

Prominence. How well-known and trusted is the business online? This is measured through reviews (quantity, recency, quality), citations across authoritative directories, backlinks to the practice website, and mentions in local publications.

Relevance. Does the business match what the patient is searching for? Google evaluates the practice's primary category, additional categories, services listed on the Google Business Profile, and the content on the practice website.

What you can control.

Prominence and relevance are the two levers independent practices can pull.

  • Reviews. Aim for five new reviews per month minimum. Ask every patient after their visit. Automate the ask via SMS or email.
  • Categories. Set your primary category to the most specific accurate match. Add secondary categories for every service you provide.
  • Services listed on your GBP. Every service should have a description written in the way patients search for it.
  • Website content. Individual pages for each major service, each with a title tag and H1 matching the search intent.
  • Citations. Consistent name, address, and phone across all major directories.
  • Photos. Fresh photos monthly. Interior, exterior, staff, equipment.
  • Posts. Weekly GBP posts announcing services, offers, or educational content.

Common mistakes that keep practices out.

  • Categories set too broadly ("Medical Clinic" instead of "Orthopedic Surgeon")
  • Duplicate Google Business Profile listings from prior locations
  • Inconsistent hours across the web
  • No photos or photos more than 12 months old
  • Suspended profile from ignored verification

Every one of these is fixable. Most take days to weeks to move rankings.
